Tirzepatide (Mounjaro®) for obesity: Information for patients from 1 April 2026
Tirzepatide, for obesity, must be prescribed alongside a reduced‑calorie diet and increased physical activity, and with referral to a wrap‑around lifestyle support service, in line with NHS England interim commissioning guidance.
Access to tirzepatide is being introduced in a phased way, over 12 years, prioritising patients with the greatest clinical need, as set out in NHS England’s interim commissioning guidance.
The drug is being made available on a phased basis, with the current group of people eligible under the national criteria being people who:
- Have a body mass index (BMI) of 35 kg/m² or more (or 32.5 kg/m² for people from minority ethnic family backgrounds)
and
- Have at least four of the following long-term conditions:1. Type 2 diabetes
2. Hypertension (high blood pressure)
3. Dyslipidaemia
4. Established cardiovascular disease
5. Obstructive sleep apnoea

Access to Mounjaro (Tirzepatide) for Type 2 Diabetes
In line with NICE technology appraisal (TA) 924, Tirzepatide is recommended as an option for treating type 2 Diabetes in adults, alongside diet and exercise, only if blood glucose is inadequately controlled and:
- Triple therapy with metformin and two other oral antidiabetic drugs is ineffective, not tolerated, or contraindicated, and
- The patient has a BMI of 35 kg/m² or more with specific psychological or other medical problems associated with obesity, or
- The patient has a BMI of less than 35 kg/m², and:
- Insulin therapy would have significant occupational implications, or
- Weight loss would benefit other significant obesity-related complications.
