How to book:
Please complete the below form to request a medical examination. A member of our team will contact you with an appointment. To secure your booking, payment is required at the time of confirmation. Once your appointment has been confirmed, we will send you a questionnaire to be completed before your appointment.

On attending your appointment please bring your last ENG 1 certificate, any hospital letters in regards to your medical conditions and some ID (passport, driving licence or discharge book).
