Update: Urgent Prescription Requests Wellbeing Practitioners – Meet our team Road Resurface 13th December
Your local NHS organisations, along with Wirral Council, have formed the Healthy Wirral Partnership and will be working with our front line staff and the public to make our local health and care services better and sustainable.
We also want to change the way in which we organise services around you. At the moment we plan services for Wirral as a whole and, when possible, we want to make services more tailored to the needs of local communities. We call this ‘Place Based Care’.

Care navigators may ask you a few questions to help navigate to the best person to help you quickly. They wont try to diagnose your problem, but you may need to tell them a little more about why you are calling.
Care navigation is a person centered approach that uses signposting and information to help primary care patients and their carers move through the health and social care system as smoothly as possible to ensure that unmet needs are met. So for example when a patient presents with symptoms that meet the access criteria for services such as pharmacists, opticians or third sector services, the acre navigator can confidently offer these choices and enable the patient to go straight to the service which best meets their health and well-being needs.
Patients get the right care, at the right time and in the right place whilst enabling their GP’s to see the more complex and long term conditions.
